The Medical Records Office provides medical report processing service, including duplication of discharge summary and selected investigation reports to our patients. The release of medical information is subject to official approval.

We will only process your request for medical report upon receipt of your documents, consent on the application form (Consent for Release of Medical Information) or insurance form and appropriate payment.
Request for Medical Reports
Guide On What You Need To Know
1) What is required during the request for medical information?
- Patient's written consent (application form)
- Patient's NRIC / Identification documents
- Medical Report Fees
- Cash payment can be made at our SOC's counter
- Cheque payment is required for request made via mail
- Cheque is to be crossed and made payable to 'NHG-Alexandra Hospital'
Note: For patients below 21 years old, the consent of the patient's parent and Patient's Birth Cerfication is required. If the patient is deceased, a copy of the patient's death certificate must be produced and the consent of the patient's estate is required.
2) How do I request for medical information?
Requests for medical information can be made in the following manner:

Apply at SOC (Clinic K or J) in person, or |

Write in permitting the hospital to release medical information for a stated purpose, or |

Submit to MRO by mail for Medical Report / insurance form / MOM form to be completed by the doctor. For example: MOM's "Medical Report for Work Injury Compensation" form, Insurance's "Hospitalisation Claim" form and "Patient Consent" form. Patient to fill in relevant portions completely before submission. |
Note: Patient's consent and the appropriate payment must be accompanied each request for medical information.
3) How soon can I obtain the requested medical information?
As a general guide, the time required for processing the medical reports is between 3 to 4 weeks. Duplicate copies of investigation results, X-ray films or medical certificate can be collected on the spot if the request is made in person at MRO.
4) How can I receive the completed medical report or Insurance Form?

You may indicate on the application form whether you want to collect the completed report personally or have it send to you via mail. Patient needs to produce identification card when collecting the report. Authorisation letter & patient's identification card is required if a third party is collecting on patient's behalf. If the patient is a decreased, please present the receipt. |
Note: A fee of S$10.00 for overseas postage is applicable.

This is a factual report compiled by a doctor base on available medical
information of the patient.

Completion of a MOM Workmen’s Compensation form
This is for the purpose of determining work-related injuries and the
degree and period of disability. Scope of the report is as per “"Medical
Report for Work Injury Compensation"” form prescribed by the Ministry of
Manpower. This may involve a review of the patient at the Specialist Outpatient Clinic.

Completion of a MOM Workmen’s Compensation Objection Form
This is for the purpose of re-determining work-related injuries and the
degree and period of disability when the patient objects to the results
of the initial workmen’s compensation report. Two specialist doctors,
different from the one who complete the initial report, will be assigned
to assess the patient and prepare the Objection report.
We also accept Objection reports from patients who had been assessed
by doctors of other hospitals for the initial report.

Specialist Medical Report
This report is prepared by a specialist doctor in response to requests
that require a professional opinion with regards to the diagnosis and
disability of the patient. It is based on an actual assessment of the
patient and may involve a review at the Specialist Outpatient Clinic.

Second Opinion Reports
This report is written for people who are not patients of Alexandra Hospital
but seek a professional opinion from our Specialists. Its format and scope
is same as the Specialist Medical Report. It is based on the assessment of
the patient and involves a review at the Specialist Outpatient Clinic.

Selective duplicate copies of investigation results such as blood test results,
lab results, X-ray and other radiological reports.

All fees are inclusive of GST and subject to change.
An administrative charge of 1/3 of the cost of the medical report fee is applicable if cancellation / withdrawal of request is made while the medical report is being processed.
Frequently Asked Questions
Can I use my Medisave to pay for the medical report?
No. Medisave cannot be used to pay for medical reports.
Can I claim the medical report fees from my insurance company?
You will need to check with your respective insurance company.
My hospitalization bill is about $200 only. How useful is the medical report for small claims?
For small claims (<S$1000), you are advised to process the insurance claim using the Inpatient Discharge Summary (provided to you upon discharge). You should request for medical reports only if the insurance company request for it.
Note: If you are applying medical reports for insurance claim, you are advised to obtain an Insurance Claim Form from your insurance agent.
If I have been seen by two different doctors in Alexandra Hospital , can I request for one consolidated medical report?
The two doctors you had visited were for two different reasons. It is necessary for
each doctor to write the medical report based on your medical condition and
treatment rendered. Hence, you need to request for two separate medical reports.
I have visited the hospital for various reasons. Which medical report should I be requesting?
Each medical report will require you to pay a fee. You are advised to request for the medical report that is relevant only for the purpose of your claim.
Question: What happen if the doctor is no longer with AH?
For doctor that is no longer with AH, we will reassigned to another doctor to write the medical report. The procedure will be the same.
Release of X-rays
With effect from 1st October 2007, our hospital has gone film-less.
Patients will have to purchase duplicated X-rays in CD format.
